Natural dentition consists of teeth that erupt into the oral cavity at different times. From the loss of a single tooth to loss of complete teeth, the position of the teeth keeps changing thus making an oral cavity as a dynamic entity. Loss of teeth in one arch with the concomitant presence of natural teeth in the opposing arch renders biomechanical issues that must be considered while designing the artificial denture. Different combinations of natural teeth and partial edentulous spaces can exist which need individual design considerations. We present a series of four clinical cases in which the maxillary arch was completely edentulous and the mandibular arch contained different Kennedy’s partial edentulous situations. The biomechanics of natural teeth in respect of occlusal plane, axial inclination and type of arch have also been discussed. All cases enable a student to visualize a case scenario and help one to decide why the base metal denture base is indicated in such case. The role of palatal vault form and the need for clinical identification of such forms has also been discussed.

Case Report
Adult-onset Still’s disease (AOSD) is a systemic inflammatory disorder. Its clinical presentation is similar to the systemic form of Juvenile idiopathic arthritis (JIA). In this case report we explore a young female 22 years after a diagnosis of JIA and 4 years after her diagnosis of AOSD who presents with systemic inflammatory symptoms. Some of the symptoms consistent with her previous diagnosis and some new. Specifically the new symptom of her diffuse lymphadenopathy lead to a differential of possible lymphoproliferative disease. After a biopsy and multiple admissions with unrelenting symptoms the patient was diagnosed with a flare of AOSD. We explore her case in depth with the question in mind: Does one diagnosed with JIA remain on long term therapy to avoid recurrent flares and progression to AOSD, and if so for how long?

Case Report
This short case-series reports a 6-month follow up of 5 rheumatoid arthritis patients (2 males, 3 females) 60 years-old or above, with Tocilizumab treatment which was discontinued due to stock off in July 2021. Patients switched treatment to other DMARD (Upadacitinib, Baricitinib, or Certolizumab pegol). We report clinical data and the Disease Activity Score for Rheumatoid Arthritis. Upadacitinib, Baricitinib, and Certolizumab appear therapeutically equivalent to Tocilizumab in a 6-month follow-up.

Baló’s concentric sclerosis is a rare and progressive demyelinating disease of the central nervous system. It is considered to be a variant of multiple sclerosis. The lesion is characterized by the alternation of concentric demyelinated and myelinated rings in the white matter while leaving the gray matter preserved. Herein we present the case of a 14-year-old female with this condition.

Case Report
It is imperative that the frontier care providers from internists and hospitalists are aware of the red flags associated with acute limb ischemia (ALI). To strategize a methodical approach for better clinical outcomes in ALI, a multidisciplinary collaboration involving internist, interventional radiologist and vascular surgeons are crucial, ideally within the first 6 hours of symptom onset. This case report discusses the clinical evaluation of a 74-years-old female presenting with ALI post an angioplasty procedure. We highlighted the clinical evaluation of ALI in respect to the importance of clinical examination, diagnostic and therapeutic imaging modalities. Additionally, we emphasized that the presentation of peripheral artery stent thrombosis in the acute setting of endovascular intervention is a serious complication that may lead to limb amputation.

This case highlights the importance of maintaining a high index of suspicion of hiatal hernia (HH) as an uncommon etiology presenting with a wide range of symptomatology as well as a masquerading left atrial mass. Further assessments with multidisciplinary imaging modalities illustrated the gastrointestinal pathology of HH.

Diffuse large B-cell lymphoma (DLBCL) is the most common non-Hodgkin lymphoma diagnosed in the United States. DLBCL often presents with constitutional symptoms, lymphadenopathy, and mass. Here we report a case of asymptomatic DLBCL with a rapidly enlarging mass as the presenting symptom. Given the patient’s history of Hodgkin lymphoma, his DLBCL is likely a secondary malignancy resulting from previous chemotherapy and radiation.
