Urinary tract infection (UTI) is one of the most common bacterial infections, affecting up to 150 million people worldwide, and certain host conditions predispose urinary tract infection to be considered complicated and therefore warrant broad-spectrum empirical treatment, in the elderly, complicated urinary tract infection should always be considered within the range of diagnoses, as the symptoms and signs for which they consult the emergency department are often very nonspecific, although this leads to overdiagnosis and overtreatment of this condition. Should not be limited to suspicion, especially if the patient has clinical conditions that assert the risk of a complicated UTI, as happened in the case of our index patient: An 83 year old adult with a history of Alzheimer's disease and urinary incontinence, admitted because he showed fever of 2 days of evolution and foul-smelling urine. A urine sample was taken for urinalysis which was suggestive of urinary tract infection and a urine culture showed infection by Citrobacter koserii. Knowing the susceptibility profile, it was decided to rotate to ciprofloxacin and continued new antibiotic treatment until completing 10 days counting as effective the days in which he received Piperacillin/tazobactam, presenting good evolution until medical discharge.

Case Report
During evolutionary stages of different living organisms, new and challenging environments has prompted the development of adaptations that may be structural, physiological and behavioral. The clinical success of denture prosthesis treatment largely rests on patient’s ability to adapt which is time dependent. While the usual outcome by most of the patients is either an adaptation or maladaptation, it is rare to observe patients adapted to extremely faulty prosthesis. This article in the form of two case reports presents unique cases of extreme adaptation to the partial denture prosthesis. Both patients had their respective partial dentures resulted in the loss of their natural abutment tooth to which they were attached. In both cases the respective abutment teeth were dangling within the prosthesis. The extent of damage to the respective abutments were to the level of the apex of the root with one case being that of maxillary canine (longest root). Removal of the denture prosthesis led to the loss of natural tooth in both cases. Psychological aspects of such extreme denture adaptation have also been discussed.

Case Report
Curcumin is an herbal supplement that is consumed for its purported anti-inflammatory, antioxidant, and antitumor properties as well as the prevention of gallstone formation. There have been few studies that investigate its role in people with current or history of biliary obstruction. Herein we discuss an encounter of a patient who presented with a perforated gallbladder after having ingested 1500-2250mg of curcumin per day for four months. We shall also review the state of known literature on the pharmacokinetic and pharmacodynamic profile of curcumin and its role as an herbal supplement. Lastly, we will examine the pitfalls of taking high levels of herbal supplements.

Breast fibroadenolipomas, otherwise known as hamartomas, are a relatively rare finding in regard to the occurrence of breast masses. Breast hamartomas make up approximately 4.8% of benign breast tumors in women. Breast hamartomas are uncommon, and our case presents with a 4.7 cm hamartoma with a focal, irregular mass inside of it which was histologically confirmed as pseudoangiomatous stromal hyperplasia (PASH). PASH is a benign mesenchymal lesion of the breast that is characterized by dense collagenous stroma forming “pseudoangiomatous” capillary-like spaces lined by spindle cells. [1] A rare condition overall, PASH is predominately discovered in premenopausal women, with fewer than 1,500 cases of PASH documented in literature [1]. Therefore, our case presentation describes a combination of two relatively rare phenomena within one palpable breast mass. Due to the absence of symptoms, the patient was scheduled for interval follow up to monitor the breast hamartoma for interval change in size, and definitive treatment by elective surgical excision was offered if accompanying symptoms developed.

Case Report
This case report describes a patient with hypothermia induced via spinal morphine and its reversal with Lorazepam, a short acting benzodiazepine. The patient presented with refractory hypothermia following Caesarean section with paradoxical vasodilation and diaphoresis. The successful prompt reversal of the hypothermia with Lorazepam while maintaining pain relief highlights the benefit of its use in cases of morphine-induced hypothermia. However large clinical trials are still needed to verify the benefits of such treatment.

Case Report
Colonoscopy is the gold standard for adenoma and adenocarcinoma detection. Here we present a rare case of a patient who presented with large bowel cancer despite a negative colonoscopy, no signs or symptoms, and a negative CT scan. This patient ordered a CEA marker only because his sister was diagnosed with colon cancer; the CEA marker came back elevated. He also had a CT scan which came back negative, with the suggestion of doing a PET scan. The PET scan finally found the suspicious mass. The patient had a bowel resection with metastatic spread to the peritoneum but all surrounding lymph nodes were negative. However, an inguinal lymph node was inflamed and later biopsied which showed metastatic carcinoma suggesting an adnexal skin carcinoma or primary urothelial carcinoma. The patient denied any skin carcinoma and also followed up with a cystoscopy which came back with normal results.
