Cytogenetic Remission in Response to Dasatinib Monotherapy in Mixed-phenotype Acute Leukemia with t(9;22)(q34.1;q11.2) Translocation
Case Report
The case of a 44-year-old Japanese woman diagnosed with mixed-phenotype acute leukemia with t(9;22)(q34.1;q11.2); BCR-ABL1 (MPAL-BA) is reported. Her white blood cell count was 5,100/μL with 13.0% blasts; positive for CD34, HLA-DR, MPO, CD33, CD19, cytoplasmic (cy) CD79a, CD22, CD10, and TdT; and negative for cyCD3. G-band cytogenetic analysis detected a normal karyotype. Fluorescence in situ hybridization detected BCR-ABL1 (9.0%) and a minor BCR-ABL1 mRNA at 4.2 × 104 copies/µg RNA. The patient was treated with dasatinib monotherapy (140 mg daily). Bone marrow examination revealed cytogenetic remission (CyCR) with minor BCR-ABL1 mRNA detected at 1.1 × 103 copies /µg RNA on day 31. No adverse events of dasatinib therapy were observed. MPAL-BA is very rare and dasatinib monotherapy efficacy for this diagnosis was previously unknown. The patient achieved CyCR with dasatinib monotherapy as first-line treatment. The present case suggests that dasatinib monotherapy may be effective and safe for MPAL-BA induction therapy.

Perforation after a Percutaneous Biopsy from Metastatic Pancreatic Cancer
Case Report
Metastatic adenocarcinoma of the pancreas has a poor prognosis. When distant metastasis occurs in the liver, image-guided percutaneous interventions are crucial for both diagnosis and treatment. In this case report, we describe a 67-year-old male with pancreatic adenocarcinoma and metastatic liver lesions. The lesions were biopsied leading to hemorrhage on post-procedure day seven.

Polyserositis as a Probable Early Manifestation of Systemic Lupus Erythematosus
Case Report
The involvement of serosas is a pathological condition shared by various diseases among which is systemic lupus erythematosus as an example of immunological condition. Polyserositis involves damage to various organs, thus overshadowing the prognosis of patients. We present the report of a case of a 49-year-old female patient with pericardial and pleural effusion of approximately 5 months of evolution, accompanied by clinical situations that justified the presence of such serositis which distracted the diagnostic process of lupus, in addition to an inconsistent elevation of antinuclear antibodies that caused a delay in treatment supported by the current criteria for diagnosis of systemic lupus erythematosus, favoring the fatal outcome of the patient. We suggest that these new diagnostic criteria for systemic lupus erythematosus prioritize greater value for clinical manifestations, since initiating the diagnostic algorithm based on laboratory criteria can lead to significant delays in the therapeutic approach with disastrous results for patients suffering from this disease.

Maggots with Methicillin Resistant Staph Aureus
Case Report
Myiasis is an infestation of the tissues and organs of living vertebrates by fly larva that feed on the host`s necrotic or living tissue, it is a very well recognized complication of neglected wounds. [1] People with neglected open wounds, poor hygiene, rural territories and close proximity with cattle are at risk of Myiasis. Here I report a case of 18 years young male who had history of injury one month back, contused lacerated wound was sutured at primary health centre, was given proper antibiotics coverage also. After stitches removal and partial healing, Patient neglected wound care and presented to me with maggots during first consultation. Diagnosed with MRSA infection, cured with surgery, culture sensitive antibiotics and regular wound care till complete healing status. He was saved from dreaded complications such as osteomyelitis, gangrene and systemic sepsis through prompt diagnosis and treatment.

Colonic B-cell Lymphoma Induced Intussusception in 18 Years Old Female, A Case Report
Case Report
Adult intussusception is a rare entity that differs from childhood intussusception. It can be challenging to diagnose due to its nonspecific presentation. In this case report we discuss an 18 year old female who presented with unspecific abdominal pain and was diagnosed with intussusception on CT. Further workup showed a mass and biopsy confirmed a lymphoma.
