Discovery of a Poly-metastatic Pulmonary Adenocarcinoma Following the Diagnosis of a Bone Lesion in the Mandible: A Case Report

Introduction: Metastases of the oral cavity are rare and concern only a tiny part of the neoplasias of the oral cavity. The detection of an atypical lesion should always alert the clinician to the possibility of primary or secondary cancerous involvement. Observation: The case presented here describes a patient presenting to the consultation for pain in the temporomandibular joint. A panoramic X-ray revealed an atypical osteolytic lesion located at the level of the left articular condyle. Additional investigations confirmed the existence of a lytic lesion indicating a metastasis or a primary sarcomatosis type. Complementary examinations made it possible to establish the diagnosis of a poly-metastatic pulmonary adenocarcinoma and to begin the management of the patient who was unaware of her condition. Conclusion: The discovery of an atypical bone lesion should alert the clinician and requires the performance of additional examinations to establish a diagnosis that can condition the survival of a patient.

Ketamine Bladder: A Cause of Obstructive Uropathy in a Young Patient
Case Report
Ketamine-associated cystitis is growing in prevalence as the availability and use of illicit drugs increases. Delayed diagnosis and management can lead to poor patient outcomes including irreversible renal impairment. Early diagnosis, ketamine cessation and management of complications is therefore crucial. There are only a few case reports in the literature describing this condition. We report an additional case of a young patient with ketamine-associated ulcerative cystitis causing upstream obstructive uropathy.

Essential Thrombocythemia Incidentally Discovered in 5-year-old Patient: A Case Report
Case Report
Essential thrombocythemia (ET) is considered a classic” BCR-ABL1-negative chronic myeloproliferative condition (MPD) that’s characterized by thrombocytosis, bleeding, and/or thrombosis. It usually affects the megakaryocytic cell lineage. While the disease is most commonly observed in people between the ages of 50 and 60, ET is an extremely rare myeloproliferative disorder in the pediatric age group. In children under the age of 14, the annual incidence of ET is estimated to be 1 per 10 million, approximately 60 times lower than older ages. Most ET patients, especially children are asymptomatic and discovered incidentally while doing investigations for other reasons. Herein, we introduce a very rare case of essential thrombocythemia in a 5-year-old male patient that was detected incidentally following receiving the influenza vaccine. The patient had a persistent elevated platelet count over 450x109 /L. Bone marrow showed markedly increased megakaryocyte number. Genetic testing and bcr/abl rearrangement were unremarkable. The patient showed no symptoms during the period of follow-up.

Acute Infectious Purpura Fulminans with Anticentromere Antibodies
Case Report
Purpura fulminans (PF) is a rare, life-threatening syndrome characterized by disseminated intravascular coagulation (DIC) and endovascular thrombosis resulting in a characteristic pattern of cutaneous purpura. There have been no reports of the combination of PF and anti-centromere antibody (ACA). A 70-year-old woman with a 2-day history of lumbago was transported from a local medical facility to our hospital due to right hydronephrosis due to ureterolithiasis and urosepsis. She had systemic lupus erythematosus (SLE) and schizophrenia. On arrival, she had consciousness disturbance and was in a shock state with cyanotic extremities. She was additionally diagnosed with renal failure, DIC and ACA positivity after examinations. She was treated with vasopressor, antibiotics and ureter stent placement. However, she developed unstable circulation, which necessitated tracheal intubation and continuous hemodiafiltration. Urine and blood cultures showed Escherichia coli. However, the cyanosis of all of her extremities changed to necrosis and she died on day 8 due to multiple organ failure. This is the first case of acute infectious PF with ACA positivity. As ACA is known to be a risk factor for digital necrosis, the present case suggests that in addition to a decrease of protein C or S, ACA may contribute to the development of PF.

Empyema Secondary to Lactobacillus Casei: A Case Report
Case Report
Lactobacilli are anaerobic or facultative, Gram-positive bacilli that commonly colonize the gastrointestinal and genitourinary tracts. The most common species of Lactobacillus are Lactobacillus casei and Lactobacillus rhamnosus. Although Lactobacillus species are generally thought to be protective and nonpathogenic microorganisms, they can lead to significant life-threatening infections in immunocompromised patients, labeling them as opportunistic pathogens. We present a case of pathogenic Lactobacilus casei causing an empyema in an immunocompromised patient, which is the first reported case of this nature.

Complete Occlusal Rehabilitation of a Grossly Decayed Natural Dentition Using Multiple Prosthodontic Options
Case Report
Complete occlusal rehabilitation involves multiple disciplines and specialties of dentistry. Within Prosthodontics it requires meticulous planning of correcting existing occlusal problems and finding new biologically acceptable occlusal parameters which include centric occlusion, occlusal plane, aesthetic position of anterior teeth and vertical dimensions. Very less occlusal rehabilitations will be considered complete if the above parameters are not biocompatible with temporomandibular joint function. This case report of a 43-year-old housewife who presented with a chief complaint of grossly decayed natural teeth is one such case where the above parameters were all corrected. While extra oral parameters were within normal limits, intra oral dental condition was corrected by a series of extraction of teeth, endodontic (intentional and conventional), surgical crown lengthening and multiple prefabricated and cast post cores. The anterior guidance was established and corrected on the temporary restorations which was then transferred to the definitive restoration. Foundation restorations were built using multiple prefabricated fiber posts and custom made cast posts. Final restorations that included single crowns and multiple fixed partial denture were fabricated of metal ceramic. A cast partial denture was also part of the overall occlusal rehabilitation. At subsequent follow up, the patient was extremely satisfied with the outcome of the treatment.
