Ischemic stricture of the small intestine is a rare complication of mesenteric vein thrombosis. We report a case of small intestine stricture that complicated an asymptomatic superior mesenteric vein thrombosis. This diagnosis was challenging owing to the silent course of the primary cause; the superior mesenteric vein thrombosis and the rarity of the ischemic stricture of the small intestine as a complication. The patient underwent resection-anastomosis and passed an uneventful postoperative course. We concluded that a high suspicion level should be maintained during dealing with the cases of intestinal obstruction due to small intestine stricture as mesenteric ischemia could be the underlying cause.

Acute Infero-Lateral MI after Blood Donation in a Patient of Myocardial Bridge: a Case Report of Rare Co-Occurrence

Case Report

The myocardial bridge (MB) is an anomaly of the coronary arteries that usually affect the left anterior descending artery (LAD). It mainly affect middle aged patients at low risk for CAD, but can manifest as unstable or stable angina, AMI and sudden death, with the latter two being rare. We report a case of 40 year-old-man, who presented with complaint of severe crushing chest pain associated with sweating and nausea just half hour after blood donation. The electrocardiography (ECG) was consistent with an acute infero-lateral myocardial infarction. Coronary angiography revealed substantial myocardial bridging in the 2nd posterolateral branch and some degree of bridging in the 1st posterolateral branch. An empirical diagnosis of symptomatic myocardial bridging with superimposed coronary vasospasm, probably precipitated by autonomic disturbance after blood donation, was made and patient discharged on oral β-blocker therapy. Although association of AMI after blood donation in a patient having myocardial bridge involving left anterior descending artery is reported, there is no reported case of such co-occurrence with right coronary artery myocardial bridge.

Case Report

Cogan's syndrome (CS) is a rare autoimmune vasculitis. Clinical hallmarks are bilateral interstitial keratitis and vestibuloauditory dysfunction. Association between Cogan's syndrome and systemic vasculitis as well as aortitis also exists. We report a case of cogan’s syndrome in 12 year old girl who intialy present with bilateral interstitial keratitis with audiovestibular and cardiac symptoms. We discuss the clinical aspect, the pathogenic mechanisms, the laboratory investigations, the differential diagnosis and traitement of cogan syndrome.

Proteus Syndrome

Case Report

This rare sporadic disease is named after the Greek god Proteus who could change shape. The syndrome has protean manifestations that include partial gigantism of the hands and feet, cerebriform plantar hyperplasia, hemangiomas, lipomas, lipohypoplasia, linear verrucous epidermal nevi, patchy dermal hypoplasia, macrocephaly, hyperostosis, muscular hypoplasia, and hypertrophy of the long bones.

Stomach Injury in Multiple and Polytrauma

Stomach injury is one of the most severe types of injuries and among severe injuries of piece abdominal injuries makes 25%. Post traumatic stomach ruptures make 1% of abdominal injuries in isolated abdominal traumas and up to 6% in combined. During 1999-2012 periods we have analyzed 57 observations, of them 12 isolated and 45 combined gastric injuries. Injuries of anterior gastric wall were revealed in 42 patients, of posterior wall in 4 persons with thoracoabdominal wounds and of both walls in 11 patients. Study proved some different surgical treatment tactics of stomach injury have been presented. Complications in the postoperative period developed in 9 (15, 8%) patients with ruptures in all layers of the stomach. Relaparatomy became necessary in 5 patients due to the following indications: ileus (2), pancreatitis (1), bleeding to gastric lumen (1) insolvency of stomach wound sutures. 3 of 5 reoperative patients died, all of them had combined traumas; 2 mortality of shock and blood loss, 1 from pancreatitis and peritonitis. An adequate intervention volume in gastric injuries appears to be two-layer suturing of the wound with previous ligating of bleeding vessels and following decompression of the organ through nasogastral probe.
