Left ventricular aneurysms are a frequent and serious complication following acute myocardial infarction and are most commonly located at the ventricular apex. The majority of these patients presents with severe mitral insufficiency, congestive heart failure, systemic embolism and sudden cardiac death. Giant aneurysms occurring in a submitral position between anterior and posterior papillary muscles on the lateral ventricular wall constitute a minor entity and those leaving the mitral apparatus intact are extremely rare. Herein, we report the case of a 70 years old male patient with a past history of inferior wall myocardial infarction 1 year back, now presented with right hemiparesis due to cardio-embolic stroke caused by thombus present in submitral left ventricular aneurysm. Despite the size of the aneurysm and its close relationship to the posterior mitral annulus the mitral apparatus was intact with a competent valve and normal left atrial size. To the best of our knowledge there are only few case reports in the literature where a patient develops large submitral LV aneurysm after myocardial infarction.desi adult stories

Scrub typhus, also called tsutsugamushi disease in Japan, is one of the most common rickettsial diseases in this country. Fever, skin rash, eschar and elevated hepatic enzymes are main clinical features of scrub typhus. We present the case of a 55 year-old woman contracted with this infection without skin eruption and abnormal liver function. The finding of an unusual eschar helped us to make a correct diagnosis. The fever subsided within 24 hours after minocyline therapy. Scrub typhus of Kuroki serotype was confirmed by indirect immunoperoxidase assay and nested polymerase chain reaction with her whole blood and eschar.abortion in the first trimester

Venous leg ulcers are the late and severe clinical form of chronic venous disease. Superficial venous system, deep venous system or perforating veins between them which are affected by reflux, obstruction or inability of the muscle pump, play an important role in pathophysiology. Compression bandages and wound care are the basis of treatment for venous ulcers. For ulcers which do not respond to these treatments, surgical procedures may be considered. There is a need for a multidisciplinary approach for the treatment and follow-up of venous leg ulcers. Cutaneous lesions like stasis ulcer are common in patients with chronic renal failure. Healing of chronic ulcer is difficult and can take time. Here we emphasized the effects of topical use of un-fractionated heparin on wound healing of an elderly male patient with chronic renal failure.adult fetish stories

Incarceration rarely complicates rectal prolapse. Even more rarely, it becomes gangrenous, necessitating emergency surgery. We describe an extremely rare case of gangrenous rectal prolapse. The patient underwent emergency perineal proctosigmoidectomy, the Altemeier operation. The postoperative course was quite uneventful with an excellent final result. The successful treatment of this patient illustrates the value of the Altemeier procedure in the difficult and unusual case scenario of bowel incarceration complicated with gangrene.why do men cheat on their girlfriends

Fenestrations and dehiscence’s are variations of the natural teeth and the alveolar bone that are significant not only to anatomy and periodontology but also to a prosthodontist who should be able to characterize such natural and normal features within his dental prosthesis. Although their etiology has been attributed to heavy occlusal forces, this article presents a case where opposing occlusion did not exist and patient's history revealed that the dehiscence was aggravated after removal of opposing teeth thus suggesting a strong relation between degree of dehiscence and lack of occlusal forces.why do men cheat on their girlfriends

Background: Angioleiomyoma of the nasal cavity is an extremely rare benign tumor. Since the first case reported by Poch-Viñals R. et al. in 1967, only 23 cases of angioleiomyoma of the nasal cavity have been published in English literature. Methodology/principal findings: We report a 42-year-old woman who presented with nasal obstruction and small volume epistaxis episodes. In the physical exam and under nasal endoscopy, we founded a red lesion that measured 1.5 cm. Clinically and in imagery the tumor resembled an inverted papilloma. Pathological study revealed an angioleiomyoma of the nasal cavity. Conclusion: Angioleiomyoma was an unusual tumor of the nasal cavity. It is more common in females. Treatment for these tumors is based on local resection, and there are no reports of recurrence after total excision.reasons wives cheat on husbands

Endoscopy and its associated clinical procedures have relatively become a routine diagnostic procedure among various medical branches especially in gastroenterology. Having both invasive and non-invasive attributes, the procedure in its present form imparts fear and apprehension among patients, especially in patients who have a hyperactive tongue or a gag reflex. This article in the form of a clinical case report describes the fabrication of an innovative diagnostic endoscopic splint which helps both operator and patient in different ways, whereas at the same time it overcomes the problems associated with such clinical procedure.adult fetish stories

Klinefelter’s Syndrome is a form of male hypogonadism which may present with; either a congenital chromosome abnormality; or alternatively with a mixture of 47XXY/46XY mosaics or higher-grade sex chromosomal aneuploidy as well as structurally abnormal X chromosomes. Clinically, the syndrome is characterized by findings of small, firm testes and symptoms of androgen deficiency but they may also present with azoospermia, tall stature and bilateral painless gynecomastia. This article will describe the presentation, investigation and eventual diagnosis of identical twins with Klinefelter’s Syndrome. Both presented with varying levels of morphological features indicative of Klinefelter’s Syndrome and required further hormonal and genetic investigation. These two cases illustrate the difference in presentation of Klinefelter’s Syndrome and the challenges experienced regularly by clinicians when attempting to treat patients diagnosed with sensitive Syndromes. This is made especially difficult when there are background issues of language barriers and compliance issues.catch a cheat
