American Journal of Medical Case Reports

Gilbert’s syndrome (GS) is a common hyperbilirubinaemia syndrome caused by reduced conjugation of serum bilirubin by the liver. Although it is considered as a common and harmless condition not requiring treatment symptoms associated with GS may be unfavorable. Here we present a case of GS where high level of total and direct bilirubin, yellowish discoloration of the sclera as well as associated symptoms including migraine, fatigue and granulomatosus dermatitis were reversed following a shift toward the popular paleolithic and then toward the paleolithic ketogenic diet.

Melanocytoma of Optic Disc: Clinical Manifestations, Imaging Characteristics and Follow Up

Case Report

Melanocytoma of the Optic nerve head, is a benign, stationary pigmented tumor present on or adjacent to the optic disc. Rarely it exhibits minor enlargement and can also undergo malignant transformation to a melanoma. Histopathologically a melanocytoma consists of intensely pigmented round to oval nevus cells with benign features. We report a case of a 77 year old lady with an Optic nerve head melanoma. The clinical features and the imaging characteristics including Ultrasound, Fluorescein angiography and Magnetic resonance imaging are presented.

A Case of Acute Myeloid Leukemia with Bilateral Leukemic Optic Nerve Infiltration

Case Report

Ocular involvement in patients with leukemia may be due to various reasons and it is characterized by various clinical symptoms. Ocular problems in patients who have acute myeloid leukaemia (AML) are commonly observed. Orbital and ocular lesions carry a poor prognosis in patients with acute myeloid leukemia. Direct invasion of the retina and choroid with the neoplastic cells is common, but the involvement of the optic nerve, uveal tract are relatively rare. Ocular symptoms include blurred vision and diplopia due to the involvement of optic nerves. This paper reports a 68 year old patient with AML. 5 azasitidin was initiated for treatment. By the second cycle, he complained of vision loss for right eye. These findings occured for left eye after three days. Optic nerve involvement was found with orbital magnetic resonance imaging with contrast. We report these patient because of rarity.

A 72-year-old Patient with Dermoid Cyst

Case Report

Introduction: It has been reported that the risk of malignant transformation is doubled in patients with mature cystic teratoma aged over 50 years. It is rare to encounter benign forms of mature cystic teratoma in elderly women. We aimed to report a 72-year-old woman with a benign cystic teratoma. A 72-year-old woman detected by ultrasonography to have a pelvic mass with a size of 14x12x10 cm was admitted to our department for detailed investigations. A mature cystic teratoma was diagnosed on the basis of transvaginal ultrasonography, magnetic resonance imaging, and serum concentrations of tumor markers. Postoperative pathology examination confirmed the diagnosis. This report once again showed that although mature cystic teratoma is rare in elderly women, it still occurs.

Disseminated Carcinomatosis of Bone Marrow Due to Sigmoid Colon Cancer

Case Report

Introduction: Disseminated carcinomatosis of the bone marrow (DCBM) caused by colorectal cancer is rarely seen. DCBM is often accompanied with diffuse intravascular coagulation (DIC). Presentation Of Case: A 72-year-old male patient with a history of right nephrectomy three decades ago for referred benign disease, underwent sigmoid colon resection for cancer causing bowel obstruction. The preoperative evaluation revealed elevated values of CA 19-9 and PSA, with no metastatic lesions in abdominal CT. On eighth postoperative day, the patient developed fever with anemia and thrombocytopenia. Bone marrow examination revealed disseminated carcinomatosis caused by colon cancer. The patient deceased on the twenty seventh postoperative day due to the rapid progression of the accompanied disseminated intravascular coagulation. Discussion: Prognosis for patients with cancer who develop disseminated carcinomatosis of bone marrow (DCBM) is poor. Conclusion: In our case, the rapid deterioration of the patient gave us no option for adjuvant chemotherapy. The outcome was fatal. A 72-year-old male patient with a history of right nephrectomy for benign disease underwent sigmoid colon resection for bowel obstruction due to cancer. Malignant Peritoneal Mesothelioma Presenting as Ascites of Unknown Origin

Case Report

Malignant mesothelioma is a rare but highly aggressive and fatal tumour arising from the mesothelial cells, which is associated with an involvement of the peritoneum in 30% of cases. We report a 48-year-old man with malignant peritoneal mesothelioma who presented with ascites of unknown origin, and discussed the clinical presentation, investigation and management, as well as the diagnostic difficulties in managing this unusual and unfortunate case.

A Case Report on Acute Lymphoblastic Leukemia in A 10 Days Old Neonate

Case Report

Although leukemia’s are the most common malignant disease seen in children, it occurs rarely in neonates. Only few cases of congenital leukemia have been reported in literature. We report here a rare case of congenital lymphoblastic leukemia in a 10 days old full term male neonate of normal home delivery who presented in the hospital with complaints of fast breathing and fever of 1 day duration. Initial clinical manifestation was that of Multi organ dysfunction syndrome due to late onset neonatal sepsis but was found to have acute leukemia on subsequent investigations.

We present the case of a 30-year-old man who sustained partial thickness burns to his right leg following the spontaneous ignition of an e-cigarette battery that was being stored in his pants pocket. As the e-cigarette industry continues to rapidly grow, we must be aware of this potential danger.
