Immune trombocytopenia is an autoimmune disease characterised by abnormal platelet destruction. Corticosteroids and intravenous immune globulin are main first line treatment options. Splenectomy is a preferred second line treatment option for unresponsive patients at many centers. Secondary thrombocytosis is a possible complication awaiting this group of patients after splenectomy. Here, we present a case of myocardial infarction at a patient who had undergone splenectomy for ITP, which is a rare event seen with secondary thrombocytosis due to splenectomy.catch a cheat

Case Report
Phaeohyphomycosis causing rhinosinusitis is not very frequent. We here report a case of rhinosinusitis caused by Fonsecaea pedrosoi in a 53 year old diabetic male, with deviated nasal septum. With surgical correction and antifungal treatment with itraconazole, the patient’s condition improved and he was discharged. Fonsecaea exists in the environment as a saprophyte and its infection is usually not lethal. However in immunocompromised individuals, when there is systemic invasion the mortality rate is high. With limited treatment options available for this fungus, early and prompt identification plays a major role not only in treatment, but also prevents further invasion.walgreens photo coupons in store

Case Report
Background: Extra ocular sebaceous carcinoma is a rare aggressive adnexal tumour. The diagnosis of this tumour is difficult due to the variety of its clinical and histopathological features. The prognosis is actually considered as aggressive as the ocular counterpart. Methods and results: We propose to discuss the clinical features, histopathological findings and aggressive potential of extra ocular sebaceous carcinoma. Therefore, five extra ocular sebaceous carcinoma cases, diagnosed at the Dermatology and Pathology departments of Farhat Hached Hospital, from January 1998 to December 2007, were reviewed. The diagnosis was based on a histopathological examination. Three of them developed locoregional and/or distant metastases. One patient died from metastatic disease. Surgery was the mainstay of treatment without any cutaneous recurrence after a mean follow-up of two years. Conclusions: Extra ocular sebaceous carcinoma has an unpredictable prognosis. It can be highly aggressive and cause death. Surgery remains the mainstay of treatment.black women white men

Atrial myxoma is a rare condition. Cardiac failure after left breast cancer treatment is not usually attributed to such pathology. We describe the case of a 69 year-old female who presented two sub-obstructive left atrium myxomas one year after the end of left breast cancer treatment. Acute cardiac failure, without history of prior cardiovascular disease, could have resolved from the combination of various factors involving myxomas and complications of radiotherapy and chemotherapy. Symmetrical peripheral gangrene (SPG) is a rare but devastating complication of septicemia, with a high mortality rate. About half of the patients who survive require amputation of the affected limb. Oral melanoma is a rare aggressive neoplasm which accounts for only 0.2-8% of all reported malignant melanomas. The common sites of its occurrence are the palate and gingiva with the maxillary arch being affected 80% of the time. Because of their presence at relatively obscure areas in the oral cavity, most of the malignant melanomas of the oral cavity are diagnosed at a late stage. We report a case of oral melanoma in a 55-year-old post-menopausal woman who had surgical excision followed by chemo-radiation but suffered recurrence. Case Report
Atypical hemolytic uremic syndrome (HUS) is most commonly due to dysregulation of the alternative complement pathway. It is characterized by the classic triad of hemolytic anemia, thrombocytopenia, and acute kidney injury. Treatment includes supportive care including hemodialysis along with steroids, plasmapheresis, and eculizumab. We report a case of a 22 year old female who was diagnosed with atypical HUS and was also noted to have a severe degree of proteinuria, nearly 24 grams in the 24 hour urine collection. Proteinuria is observed in cases of atypical HUS because of the glomerular leakage of protein due to severe endothelial and epithelial injury. However, there is little data directly addressing the degree of proteinuria observed in these patients. A 52 year old male with history of alcohol abuse presented to the hospital with metabolic acidosis, he reported consuming a bottle of Listerine for the past two months. On admission he high anion gap and osmolar gap. Given the severe acidosis the patient was started on a bicarbonate drip and also Fomepizole. Later the assays for ethylene glycol and methanol came back negative. Case Report
The amount of foreign bodies that gains access to the Gastrointestinal Tract (GIT) on yearly basis is in millions. These are done by the two main routes to the tract, i. e the mouth and the anus. Ingestion of foreign bodies may be harmless but sometimes death in the region of 1000 – 3000 yearly may also happen. The vast majority of ingested foreign bodies into the GIT pass through the tract without significant incidence. Affected individuals may be going about with their normal daily activities without any form of bodily interruption. Up to 20 % of cases however will need some form of therapeutic intervention. 80 % of cases of ingestion of foreign bodies into the GIT have been shown to occur in children. Anal insertion of foreign bodies into the GIT on the other hand occurs most often in the adult age group. Foreign bodies in the GIT may also be seen in the psychiatric patients, inmates, chronic alcoholics, drug abusers, the elderly patients with poor fitting dentures as well as the mentally retarded individuals. While ingestion of foreign bodies into the GIT may be accidental (especially in children), or experimental (anal insertion by adults), it may be purely intentional as in smugglers of illicit drugs, jewels and other valuables so as to evade detection by the security.phone spying
