Unusual presentation of Gastrointestinal Basidiobolomycosis in a 7-year-old Child – Case Report

Gastrointestinal basidiobolomycosis (GIB) is a rare fungal infection caused by basidiobolus ranarum. It has been recognized increasingly in the southern region of Saudi Arabia. Here, we report an unusual case of pediatric GIB with anorectal involvement in a 7-year-old Yemeni boy living in the south of Saudi Arabia. He had abdominal pain, abdominal distension, rectal bleeding, weight loss, perianal swelling and redness, and peripheral eosinophilia. The abdominal computed tomographic (CT) scan revealed significant wall thickness of the rectum. Histopathological findings of full thickness biopsy were consistent with basidiobolomycosis. The patient was treated with voriconazole and showed marked improvement. This case highlights the importance of considering GIB in children presenting with bleeding per rectum and eosinophilia associated with anorectal mass.

Rheumatoid arthritis (RA) is a well-known auto-immune disease affecting the joints, its signature signs being bone erosions and joint deformity. Other than arthritis, RA can also affect other organs in the body, such as eyes, heart, lungs, etc. In our case we are going to discuss an elderly lady with RA, presenting with large pleural effusion as presenting complaint.

A Case of African Visceral Leishmaniasis (VL) Treated with a Combination of Liposomal Amphotericin-B and oral Miltefosine
Case Study
A 36-year-old Nigerian man was assessed for chronic recurrent fever, generalized weakness, body aches, epistaxis and bleeding gums of 3-month duration. Despite receiving treatment for suspected malaria twice and undergoing multiple courses of antibiotics for suspected infections, his symptoms persisted. Examination revealed hepatosplenomegaly, lymphadenopathy, and severe pallor. Investigations revealed severe anemia, pancytopenia, raised globulin levels. Bone marrow biopsy revealed Leishmania Donovani (LD) bodies. He received treatment with a short duration of high-dose liposomal Amphotericin B followed by outpatient oral Miltefosine to which he responded successfully.

Left Atrial Myxoma: A Case Report and Review of Literature

Primary cardiac tumors are an infrequent cause of symptoms mimicking valvular heart disease, congestive heart failure, arrhythmias, conduction defects and pulmonary disease. The rarity of this condition can lead to significant delay and cause patient distress. Here, we present a case of an unusual prolapsing left atrial myxoma in a 61-year-old female who presented with several months of dyspnea.  The patient endorsed weight loss and night sweats during this period and treatment with antibiotics and steroids for pulmonary symptoms had not provided relief. Seven months after her initial presentation, an echocardiogram was performed revealing a 2.85 x 5.44 cm left atrial mass on the interatrial septum. The mass was resected by cardiothoracic surgery with complete resolution of her symptoms.

Don’t Forget to Take Your Vitamins! A Case of Gastric Bypass Gone Wrong
Case Report
Gastric bypass, an increasingly common operation performed due to rises in obesity has greatly improved patient’s quality of life along with health. This case report presents a 62 year old female three decades post Roux-En-Y gastric bypass who came in with shortness of breath. Knowing the malabsorptive nature of her weight-loss surgery we suspected a nutritional deficiency and performed a full panel. Though we found multiple vitamin abnormalities, we were most concerned about her deficient thiamine levels. The complete workup led to a diagnosis of wet beriberi, a condition commonly seen in underdeveloped nations. With the rise in weight loss surgery, understanding and having a high clinical suspicion for such patients is necessary.

Rifabutin low dose-induced intermediate uveitis in a patient with Acquired Immunodeficiency Syndrome
Case Report
Intermediate uveitis is a relatively rare manifestation of Rifabutin induced uveitis. Rifabutin toxicity is unusual at doses of 300 mg daily or less. The purpose of this study is to report the case of an HIV+ patient with unilateral rifabutin induced intermediate uveitis after 4 months of treatment with low dose rifabutin (150 mg daily) in combination with ritonavir and fluconazole. The uveitis resolved after drug discontinuance and the use topical corticosteroids.

Calciphylaxis Masquerading as Warfarin Induced Skin Necrosis
Case Report
Calciphylaxis is rare complication of end stage renal disease on hemodialysis. Calciphylaxis closely mimics warfarin induced skin necrosis in dialysis patients who are being started on coumadin thus posing a diagnostic challenge. Skin biopsy may be required in such cases to confirm the diagnosis.

Left Varicocele Revealing a Nutcracker Syndrome: A Case Report and Review of Literature
Case Report
Nutcracker syndrome (NCS) is a rare entity defined by the symptomatic hypertension of the left renal vein (LRV) compressed in the majority of cases between the superior mesenteric artery and the aorta. Hematuria, pelvic or back pain and left varicocele are the most commonly symptoms. Doppler ultrasound, computed tomography (CT) and selective left renal vein phlebography are useful to confirm the diagnosis. Treatment criteria are not well established, however for asymptomatic patients with no haematuria, the treatment remains conservative. We report the case of a 17-year-old man, with an intermittent left flank pain for 4 years and increasing volume of the left testicle for 8 months.
