Post-partum Spontaneous Coronary Artery Dissection: A Case Report
Case Report
Spontaneous coronary artery dissection (SCAD) is a rare illness often misdiagnosed, that accounts for up to 4% of cases of acute coronary syndrome in young women. Interestingly, SCAD is the most common cause of myocardial infarction related to pregnancy. Here, we present a case of a 35-year old postpartum patient who presented in cardiac arrest due to ventricular fibrillation and was found to have a ST-segment elevation myocardial infarction. Cardiac catheterization revealed significant coronary vasospasm and dissection of the right circumflex and distal left anterior descending artery. We discuss clinical diagnosis and management of spontaneous coronary dissection along with literature review.

Lambl’s Excrescences – Taking the Crypto out of Cryptogenic Stroke: A Case Report and Literature Review
Case Report
Lambl's excrescences (LE) are thin and long filiform projections arising from the line of closure of cardiac valves. Their etiology is not well understood, but they’re thought to result from a wear-and-tear process. They are typically asymptomatic and best detected by transesophageal echocardiogram. Although controversial, some studies suggest an association between LE, ischemic stroke, and headaches. We describe a 56-year-old man who presented with a 3-day history of bilateral blurry vision within the left visual field. One day prior to his visual symptoms, he developed a severe headache that persisted until his presentation to our hospital. On physical examination, visual acuity and fields were intact, but he reported blurriness in the left visual field of both eyes. Brain MRI showed a wedge-shaped area of restricted diffusion along the medial aspect of the right occipital lobe, indicating an ischemic stroke of the visual cortex. Due to the embolic appearance of the stroke, transesophageal echocardiogram was performed and showed a LE on the aortic valve. No other embolic source was identified. In the absence of another explanation for his stroke, LE was considered the most likely source of his embolic stroke. The patient was treated with antiplatelet therapy. In conclusion, this case report highlights LE as a possible cardioembolic source of stroke and discusses its association with headache. We also provide a review of the current literature on LE and the lack of clear guidelines on its management.

Shingles Radiculoplexoneuropathy

We report on a 59 year old man with background history of hypertension who presented to our clinic with vesicular rash of two days duration extending from the right deltoid region to the dorsum of the fingers. He was treated with Valacyclovir plus oral steroid and Tramadol after a clinical diagnosis of shingles; he represented one week later with worsening pain. At that time the vesicular rash were crusting and dry. He was diagnosed with post herpetic neuralgia and his pain medication dosage was adjusted. He represented again two weeks after the initial appearance of rash with complaints of weakness in the right upper extremity. Physical examination confirmed the weakness in the proximal and distal muscle groups of the right arm associated with weak hand and finger grips. He was then referred for electromyography and nerve conduction studies. The result showed the right ulnar motor response recording at abductor digit minimi (ADM) and first dorsal interosseous (FDI) had prolonged latency, reduced amplitude and slowed conduction velocity. The findings are most consistent with right radiculoplexoneuropathy affecting predominantly lower plexus. (EMG/NCS table). He was referred for physical therapy and regained full functions of the right upper extremity after 6 weeks of therapy. Segmental motor weakness secondary to Varicella Zoster infection is uncommon; as a result, clinicians may not suspect it and it may be confused with Parsonage-Turner syndrome, brachial plexus syndrome, cervical radiculopathy and myelopathy particularly if the rash is not obvious or absent leading to delayed diagnosis and or investigations.

A Case Report: Painless Type A Aortic Dissection with Cardiac Tamponade as Initial Presentation

A 70 years old male with past medical history of hypertension, remote history of colonic and prostate cancer and ischemic stroke came to the hospital with generalized weakness, fatigue, increased shortness of breath and decreased exercise tolerance for the past week. Bedside echocardiogram revealed large amount of pericardial effusion with signs of pericardial tamponade. Patient successfully underwent echocardiography-guided pericardiocentesis with improvement of symptoms and remained stable and 900 cc of bloody fluid was drained. Blood tests revealed markedly elevated D-dimer, elevated liver enzymes and acute kidney injury. Chest X-ray showed widened mediastinum. CT chest without a contrast indicative of ascending aortic dissection, was confirmed by magnetic resonance imaging (MRI) of the chest. During and prior to admission to our hospital patient had reported no chest pain. Cardiac tamponade was the only clinical finding, which is very unusual for acute aortic dissection to be diagnosed. Patient was transferred to another institution for surgical intervention, which revealed sealed ruptured aortic dissection into pericardium, with successful repair of the dissection, and later was discharged in stable condition.

An Isolated Lytic Bone Image with Initial Normocalcemia Revealing Hyperparathyroidism

Brown tumors are rarely the presenting feature of the hyperparathyroidism and are usually accompanied by a disturbance of the calcium-phosphate balance. We report the case of a 46-year-old patient with no medical history, presenting a solitary bone lytic lesion linked to primary hyperparathyroidism. Phosphate and calcium balance was within normal range initially thus resulting in delayed diagnosis. Bone lesion has subsequently been confirmed to be a brown tumor. In front of solitary lytic bone lesion and even if phosphate and calcium balance was normal; the dosage of parathyroid hormone must be performed.

A Dento – Surgical Management of a Cleidocranial Dysplasia Case. A New Approach for Better Oral Health Quality of Life Part II to: (A 9-year-old Saudi Boy with Cliedocranial Dysplasia)
Case Report
Background:Cleidocranial dysplasia is a rare congenital defect of autosomal dominant heritable disease with prevalence of 1 per 1,000000 individuals. It is primarily affects bones which undergo intramembranous ossification [1,2,3,4]. Case presentation:A 9 years old Saudi male with CCD attended Pediatric Dental Clinics in Dammam Medical Complex. Chief complaint was delay eruption of permanent teeth and multiple caries. Based on clinical examination, radiographic investigations including CBCT & with coordination between Pediatric Dentistry, maxillofacial surgery and Orthodontic departments, a treatment plan has been concluded taking into consideration the wellbeing and the oral health related life style of the patient by addressing a new approach to preserve all the primary teeth, surgically removing all supernumerary teeth, applying removal or fixed modified Gropers’ appliance to improve speech, esthetic & function than observing the eruption of the permanent teeth to intervene orthodontically only when needed. Conclusion: Team work involving multi-disciplinary departments helped to select a preeminent & new dental management approach for CCD patients including all aspects needed, Dental rehabilitation, Surgical & Orthodontic treatment to provide better oral health quality of life for the young patient.
