Identifying the vascular anatomical variations of the operating region is essential in carrying out successful surgery, and decreasing the rate of both intraoperative and postoperative complications. Surgeons must keep in mind that arterial variation may be present in the vascular structures intraoperatively, even though it was not revealed in preoperative imaging. The present study report a case of a 51 year old woman who was diagnosed with pancreatic ductal adenocarcinoma (PDAC) with a rare variation of celiaco-mesenteric trunk originating at the level of L1. The patient underwent pancreaticoduodenectomy. The postoperative course was favorable and the patient was discharged on postoperative day 15.

Case Report
Protein S deficiency is a rare thrombophilia, autosomal dominant disorder leading to VTE especially thrombosis at an uncommon place. Arterial thrombosis and strokes are very rare presentations. We are presenting a case of 19 years old lady with Beckwith Weidemann syndrome who presented with arterial thrombosis with a stroke one month prior to thrombosis. Patient was successfully managed with intravenous heparin and thrombectomy and discharged on Apixaban for life long treatment.

Case Report
Salmonella infection in humans can present in different ways: enteric fever (typhoid or paratyphoid), acute gastroenteritis, bacteremia and local infection. Salmonella can cause vascular infection leading to mycotic aortic aneurysm. In case of vascular infection early recognition, debridement of infected tissue, endovascular repair and antibiotic use is corner stone of management. We report a case of 68 year old male who has abdominal aortic pseudoaneurysm with aortic valve infective endocarditis due to salmonella. Patient was successfully treated with tissue debridement, endovascular repair with stent placement and antibiotic use. Mycotic aortic aneurysm of abdominal aorta with aortic valve infective endocarditis due to salmonella has rarely been reported.

Although pembrolizumab is the mainstay treatment in metastatic malignant melanoma and have shown a durable response. Heterogeneous or no response has been observed in 30-40% of the cases of metastatic malignant melanoma. Metastatic lung lesions are sensitive to checkpoint inhibitor therapy whereas liver and adrenal metastases show a variable response. We are reporting a case of malignant melanoma with numerous liver and adrenal metastases showing a significant and durable response to just a single dose of pembrolizumab.

Case Report
Struma ovarii represents a rare type of teratoma that is formed entirely of thyroid tissue. In most cases, these tumors are benign; however, malignant ones have also been reported. It is usually seen between 40 and 60 years and accounts for about 5 % of all cases of ovarian teratomas. Our case was a female patient, 40 years old, who was presented with vaginal bleeding, abdominal swelling and left sided pelvic mass for six months. Clinical examination and imaging techniques showed left ovarian neoplastic lesion and small amount of fluid in Douglas pouch. After laparoscopic excision, Histopathological examination was performed, it showed ovarian tissue, predominantly formed of mature thyroid tissue which are the key features of struma ovarii. In Conclusion: Struma ovarii is a rare ovarian tumor with variable clinical presentations, even can be malignant. Pre-operative diagnosis is usually difficult and confusing, so, it should be put in consideration in differential diagnosis of ovarian masses.
