An Interesting Case of Gastrointestinal Bleeding Resulting from Dieulafoy’s Lesion
Case Report
Dieulafoy’s lesion (DL) is a relatively rare, but potentially life-threatening condition. It accounts for 1-2% of acute Gastrointestinal (GI) bleeding. DL is an uncommon, but important cause of gastrointestinal bleeding in which hemorrhage occurs from a pinpoint, non-ulcerated arterial lesion. [1] DLs are usually located in the stomach, most commonly in people between the ages 50 and 70 years. [2] We present a case of GI bleeding resulting from Dieulafoy lesion of the stomach, successfully treated with endoscopic intervention.

A Rare Case of Primary CNS Germinoma in the Pineal Region with Long-standing Hearing Loss
Case Report
Germinomas in the brain are rare neoplasms which make up less than 1% of all intracranial tumors. Their clinical presentation is usually related to the increased intracranial pressure and/or hydrocephalus. This report describes a case who presented to our hospital for headaches and visual changes, including Parinaud's syndrome, that had started recently. The patient also had been complaining of chronic bilateral hearing loss for several years which seemed not properly evaluated previously. MRI brain showed a markedly enhancing lesion in the pineal region. CSF and serum tumor markers including B-human chorionic gonadotropin (B-HCG) and alfa-fetoprotein (AFP) were normal. Lumbar puncture with cytology was negative for malignancy. Biopsy was taken from the mass and sent to the pathology lab for testing. Histopathological examination showed changes consistent with germinoma and the immunohistochemical staining including that for placental alkaline phosphatase (PLAP) confirmed the diagnosis. There was no evidence of metastasis and the patient was treated with radiation-alone therapy that resulted in marked regression of the mass and significant clinical improvement except for the issue of hearing loss which had persisted, as expected, given its long-standing existance. In addition to highlighting the diagnostic evaluation and treatment approach for such rare tumors, this case suggests considering pineal germinomas among the differential diagnosis when dealing with hearing loss which needs careful evaluation and should never be underestimated.

An Interesting Non-Radiation Cause of Cystitis in a Patient Followed with a Pre-Diagnosis of Radiation Cystitis: Surgical Suture Material, Prolene
Case Report
Radiation cystitis is a condition caused by urinary bladder injury due to radiation, and is characterized by a need to urinate often and a burning sensation when urinating. A patient who was receiving radiotherapy after being operated on for cervical cancer developed intense cystitis symptoms during treatment that continued for a long time that indicated radiation association. A cystoscopy was decided upon due to the long-term symptoms, and after the removal of a suture detected in the bladder, all symptoms resolved. The tendency to associate problems with radiation in patients receiving radiotherapy may sometimes mask the actual cause. This report aims to draw attention to such situations, and to present an interesting case of cystitis that developed due to a non-radiation cause.

Traumatic Brain Injury: a Case Report and Its Contribution to Understanding the Underlying Mechanisms - Alpha-Emitting Nanoparticulates Proven as Key

This case report on a mild traumatic brain injury (self-diagnosis of the author on his own brain) leads to alpha-emitting nanoparticulates whose major contribution to TBI is confirmed by the extremely strong occurrence of TBIs in war veterans (with wideranging administrative data available to confirm the extremely strong typicality of the case). This allows a better and clearer understanding of the secondary injury events following TBI such as blood-brain barrier damage, mitochondria dysfunction and massive neuron death, of the various neurodegenerative disorders usually diagnosed following TBI such as chronic traumatic encephalopathy, chronic traumatic brain inflammation and indirect traumatic optic neuropathy, and of the risks of ischemic stroke following TBI.

Femur Fracture Caused by Falling of a Boy with Osteogenesis Imperfecta
Case Report
Osteogenesis imperfecta (OI) is the most common hereditable cause of bone fragility in children. A Japanese 2-year-old boy fell accidentally from a slide, sustaining a femur shaft fracture. Several maternal family members had experienced episodes of hearing impairment and bone fracture. The injury mechanism was clear and competent. Child abuse was unlikely. Strong family history and presence of blue sclera confirmed the OI diagnosis. Any doctor treating pediatric femur fractures should recall the rare cause of OI. Clinical evaluation alone without a family interview might not complete the diagnosis with OI. We continued proposing to his parents the benefit of receiving bisphosphonate treatment.
