Oral Pemphigus Vulgaris: A Case Report and Literature Review
Case Report
Pemphigus vulgaris (PV) is a rare chronic autoimmune mucocutaneous blistering disease characterized by the production of autoantibodies targeting desmogleins, key components of intercellular adhesion of keratinocytes. This process leads to the loss of intercellular adhesion, culminating in the development of intraepithelial bullae and erosions. PV manifest as intraoral lesions that subsequently spread to other mucous membranes and the skin. While the exact etiology remains elusive, several triggering factors have been identified. Oral lesions of PV are often misdiagnosed and mistreated, emphasizing the need for early recognition by dental professionals. This case report aims to contribute to the existing literature by illustrating representative clinical features of PV, diagnostic approach, and therapeutic challenges associated with our case.

Focal Takotsubo Cardiomyopathy: A Case Report and Literature Review
Case Report
Takotsubo cardiomyopathy (TCC) or stress-induced cardiomyopathy, was first discovered in Japan in 1990 and is derived from the name “octopus pod” which describes the typical LV apical ballooning with a narrow base. The pathophysiology behind takotsubo remains incompletely understood. However, TCC association with conditions of catecholamine excess such as emotional or physical stress is well studied and is the most favored theory thus far. Atypical, rare forms of TCC such as mid-ventricular, basal, focal, and apical types have been identified with an incidence of 14.6%, 2.2%, 1.5%, and 81.7% respectively. We present the case of a 48-year-old Caucasian female admitted status post seizure and drowning incident who was hemodynamically stable and with a physical examination grossly unremarkable on presentation. Her echocardiogram showed hypokinesis and regional wall motion abnormality involving the anterior septal and lateral walls. Her cardiac magnetic resonance imaging showed hypokinesia of the anterolateral mid to basal wall consistent with focal Takobsubo cardiomyopathy. The patient underwent cardiac catheterization, which showed non-obstructive coronary artery disease with anterolateral dyskinesis. She was discharged with close follow-up as an outpatient.

Diverse Labial Flange Designs to Accomplish Lip Aesthetics in Conventional Complete Denture Prosthesis
Case Study
This case series describes the multiple labial flange design to overcome the problem of providing adequate labial fullness by a complete denture prosthesis in patients who cannot due to some reasons undergo alveoloplasty of the maxillary residual alveolar ridge. The relation between the design and the particular case scenario has been presented under different denture flange design. Four different design types with case presentation include that of a gumfit denture, gumfit with stainless steel wire, gumfit with wire and acrylic and finally a flanged labial window complete denture prosthesis. Each case is different and covers multiple clinical variations of providing labial support using labial denture flange without totally constructing the labial flange. Each individual design has been discussed in the light of lip relation, the height of contour of the residual alveolar ridge, the labial undercut and finally the vestibular depth. These designs are simple and mostly done in the laboratory and can be customized for all patients depending upon the availability or non availability of the bone.

The Effects of Remimazolam and Propofol on Hemodynamics, Bispectral Index and Postoperative Cognitive Function in Elderly Patients Undergoing Neurointerventional Operation

Previous studies have shown that remimazolam has a lesser effect on hemodynamics and postoperative cognitive function than propofol. In this study, we recruited sixty elderly patients undergoing neurointerventional operation under remimazolam or propofol anesthesia. The results showed that in comparison with propofol, remimazolam provided more stable hemodynamics but higher bispectral index during anesthesia induction. Patients' cognitive function declined slightly after operation, but did not differ between the two anesthetic protocols.
